Health Questionnaire
Each new student must submit a completed medical questionnaire to the Student Health Office during or before class registration. Returning students must complete an annual update each year they attend Corban. Students and employees alike can be assured that all medical records and background information are kept in strictest confidence. All students must be covered by a health or accident insurance.

Student Health Insurance
The University automatically issues medical polices for all full-time undergraduate students. The annual premium of $532 (under age 26) or $812 (age 26 and over) is added to your tuition account. If you are a current, full-time undergraduate student and have your own insurance, you need to waive the policy before September 30 of the current year. Otherwise, you will automatically receive the insurance and the premium will be added to your tuition account. Online students are exempt. To waive the insurance or to view the plan brochure, go to www.maksin.com/Corban.aspx.

Physician Referrals
The Student Health Office maintains a list of local physicians willing to see Corban students. The nurse can assist students in scheduling appointments with a physician as needed.
