Our Commitment
The Student Health Office is committed to promoting physical and mental well-being for all students and employees at Corban College.

Location
Second Floor, Schimmel Hall, Room. 217. This is a drop-in outpatient medical office provided for all registered students, spouses, faculty, staff, and campus visitors.

Health Questionnaire
Each new student must submit a completed medical questionnaire to the Student Health Office during or before class registration. Returning students must complete an annual update each year they attend Corban. Students and employees alike can be assured that all medical records and background information are kept in strictest confidence. All students must be covered by a health or accident insurance.

Student health insurance is available. To view the brochure and/or enroll visit www.maksin.com then double click "college students" and fill-in "Corban College" when prompted.

Enrollment can be completed online with a credit card. You may also download the enrollment card, print and remit with check as directed on the enrollment card. Print out the confirmation page and send a copy to the Corban Health Office.

Physician Referrals
The Student Health Office maintains a list of local physicians willing to see Corban students. The nurse can assist students in scheduling appointments with a physician as needed.
